Assessment
Individual time with one of our specialist assessors provides you with an opportunity to discuss your needs and aspirations and to receive independent advice on product choice and your ability to be safely mobile.
Types of assessments
Our range of assessment appointments are designed to suit car drivers, vocational drivers (e.g. buses and lorries), car passengers, wheelchair users and scooter users.
Booking
Anyone can attend without a referral. Assessments are booked by appointment so that we can give you the time you need.
Please click on the appropriate assessment type and print the application form at the bottom, you will then need to call us on 020 8770 1151 or email info@mobility-qe.org for the cost of the assessment and to ensure this is the correct assessment to meet your needs. Please note that we cannot accept application forms via email. Alternatively we can send you an application form.

Following your visit we will prepare and supply a written report to assist you.
Referral
If you are referred by an organisation such as the Driver and Vehicle Licensing Authority (DVLA), Solicitor or Insurance Company, you will have a more detailed assessment and a comprehensive report, which suits the referrer’s requirements. Please advise us of the referring agency at the time of booking.
